Implantologia a Carico Immediato

Il carico immediato e il carico differito sono due concetti legati all’universo dell’Odontoiatria. Molti non sono a conoscenza del loro reale significato, perché magari hanno avuto la fortuna di non dover mai avere a che fare con i problemi del cavo orale che riguardano questi concetti. Per essere più precisi essi definiscono due distinte metodologie curative per la patologia dell’edentulismo parziale o totale. I pazienti affetti da questa malattia, che in realtà è la conseguenza di altre patologie del cavo orale, hanno la necessità fisiologica e estetica di ripristinare in fretta gli elementi dentali ormai andati perduti. Fisiologica perché i problemi della bocca portano velocemente ad una degenerazione di funzioni quali la masticazione, la fonazione, la respirazione e la digestione. Estetica perché il concetto del bel sorriso è ormai un elemento imprescindibile nella nostra società, che se non rispettato può portate l’individuo ad una solitudine coatta, per via delle difficoltà nei rapporti interpersonali.

La branca dell’Odontoiatria che si occupa proprio di ristabilire il corretto funzionamento fisiologico ed estetico è l’Implantologia, tesa alla ricostruzione degli elementi dentali perduti attraverso l’impianto di materiali esterni. L’intuizione di questa branca medica non è nuova, e ci sono testimonianze di civiltà molto antiche che già avevano sentito la necessità di una sostituzione dei denti. Naturalmente le scarse conoscenze circa materiali e metodologie di intervento rendevano poco efficaci i tentativi degli antichi popoli di raggiungere lo scopo. Il moderno sviluppo di tecniche e conoscenze ha portato all’individuazione di materiali in grado di evitare fenomeni del tutto negativi per il paziente, quali possono essere quelli di rigetto da parte dell’organismo degli elementi estranei inseriti. Si deve agli studi del Dott. Branemark l’intuizione della scelta del titanio, quale materiale dalle enormi capacità osteointegrative e biocompatibili. Da allora con il titanio vengono realizzati i perni da inserire all’interno dell’osso, dove poi ancorare i più svariati tipi di protesi (dipendentemente dal caso clinico riscontrato). Partendo da questo assunto arriviamo alla spiegazione e alla diversificazione delle tecniche implantologiche con carico immediato e carico differito.

Esistono due diverse scuole di pensiero, entrambe molto valide ed autorevoli. Parliamo della Scuola Svedese e della Scuola Italiana. Alla prima si devono gli studi e lo sviluppo delle tecniche di carico immediato. Sostanzialmente si tratta dell’inserimento del perno in titanio e dell’ancoraggio della protesi nell’arco della stessa operazione. Va da sé che questa tecnica si avvale di tutte le conoscenze circa l’osteointegrazione e la biocompatibilità dei materiali utilizzati. Il vantaggio per il paziente è proprio la velocità con cui si pone rimedio ai suoi problemi di edentulismo. La scuola Italiana ha invece sviluppato la tecnica del carico differito, che distingue cioè l’operazione per l’inserimento del perno da quella per l’ancoraggio della protesi, effettuata circa 3-6 mesi dopo il primo intervento. Lo scopo è quello di permettere un monitoraggio migliore dei risultati raggiunti durante l’inserimento del perno, constatando così il livello di osteointegrazione dell’elemento. Le due tecniche sono entrambe molto valide; è pur vero però che l'implantologia a carico immediato ha recentemente destato un maggior interesse tra i professionisti medici e i pazienti stessi. Innanzitutto per la velocità con cui permette la risoluzione dei vari problemi, poi proprio per le conoscenze sulla biocompatibilità dei materiali odierne, che ne rendono più sicura ed efficace l’applicazione rispetto a qualche anno fa.

Immediate loading and delayed loading are two concepts related to the universe of Dentistry. Many are not aware of their real meaning, because maybe they have been lucky enough to never have to deal with oral problems concerning these concepts. To be more precise, they define two distinct curative methods for the disease partial or total edentulism. Patients suffering from this disease, which is actually the consequence of other diseases of the oral cavity, have the physiological needs and aesthetic restore quickly dental elements now been lost. Physiological because the mouth problems quickly lead to a degeneration of functions such as mastication, phonation, respiration and digestion. Aesthetic because the concept of the beautiful smile is now an essential element in our society, which if not respected can bring the individual to a forced solitude, due to difficulties in interpersonal relationships.

The branch of Dentistry that takes care to restore the correct physiological and aesthetic operation is the Implantology, aimed at the reconstruction of teeth lost through the external materials plant. The intuition of this branch of medicine is not new, and there are testimonies of ancient civilizations that had already felt the need of a tooth replacement. Of course the lack of knowledge about materials and methods of intervention made ineffective attempts of the ancient people to achieve the purpose. The modern development of techniques and knowledge has led to the identification of materials able to entirely avoid negative phenomena for the patient, which may be those of rejection by the body of the inserted foreign elements. With this in mind we come to the explanation and diversification of implant techniques with immediate loading and delayed loading.

There are two different schools of thought, both very valid and authoritative. We talk about the Swedish school and the Italian school. At first you will need the studies and the development of immediate loading techniques. Basically it is the insertion of titanium pin and anchor the prosthesis during the same operation. It goes without saying that this technique makes use of all the knowledge about the osseointegration and biocompatibility of materials used. The advantage to the patient is precisely the speed with which can be remedied to its edentulism problems. The Italian school has instead developed the technique of deferred load, which distinguishes that the operation for the insertion of the pin from that for the anchorage of the prosthesis, carried out about 3-6 months after the first intervention. The purpose is to allow a better monitoring of the results achieved during the insertion of the pin, so noting the level of osteointegration of the element. The two techniques are both very valid; it is also true, however, that the immediate load implants has recently attracted greater interest among medical professionals and patients themselves. First, for the speed with which it allows the resolution of several problems, then precisely because of the knowledge on the biocompatibility of today's materials, which make it more safe and effective application than a few years ago.
